Fertility in
trans people
Fertility in trans people may be affected by surgical or hormone-based treatments required for gender transition.
In this regard, at Equipo Juana Crespo we believe that the medical community has a very important role in informing patients about the impact these treatments may have on future fertility, and in providing real solutions through effective fertility-preservation treatments—without gender dysphoria or emotional impact—so that transgender people receive support across the full spectrum of reproductive health.

Fertility preservation in transgender
people: Why is it so important?
All treatments aimed at gender transition that are irreversible or partially reversible—such as hormone therapy—have an impact on fertility. Therefore, the ideal approach is to plan fertility before starting any treatment.
At Equipo Juana Crespo, we know that fertility planning in trans people goes beyond simple fertility preservation through egg or sperm freezing. There are different types of fertility preservation for trans people, and depending on your situation, we will advise you on the best treatment for your case and support you throughout the entire process.
When we talk about preservation options, in the case of transgender women this may include TTT (testicular tissue cryopreservation), sperm sample cryopreservation, or TESE (testicular sperm extraction) followed by sperm cryopreservation. On the other hand, for transgender men, egg preservation becomes the best solution.

Why Equipo
Juana Crespo?
Personalized strategy adapted to each patient’s needs
We are the only clinic where you will receive a personalized plan that includes a treatment strategy adapted to the stage you are currently in. Our team, trained and experienced in caring for LGBTQ+ patients, will support and advise you at all times.
We adapt protocols and procedures to minimize distress and gender dysphoria.
Throughout the treatment, we use alternative methods to options that may cause gender dysphoria, such as transvaginal ultrasound; these include abdominal ultrasound, transabdominal monitoring, or remote ultrasound probes. We also offer vibratory stimulation or electroejaculation, sperm aspiration, or microsurgical sperm extraction directly from parts of the testicles or the epididymis, with the aim of avoiding the use of the genitals for sample collection.
Leading results in high-complexity fertility treatments
An entire team focused on achieving the best possible outcomes—our commitment is to help you build a family, whatever your situation. This requires a high level of specialization in every area, from consultations to the laboratory and the operating room.
FAQS
Is it necessary to pause hormone treatments?
Testosterone treatments can directly affect ovarian reserve. It is necessary to pause treatment to reverse this situation. However, it is advisable to perform egg freezing before starting testosterone treatment to ensure egg quality.
When should you start considering trans fertility?
To preserve the fertility of a transgender person, it is important to address the issue as early as possible, especially before undergoing any gender reassignment surgery.
For prepubescent transgender girls, the only fertility preservation option at this stage of development is CTT, or cryopreservation of testicular tissue. In later cases, other options exist.
Is fertility preservation essential for trans men?
As we explained a few lines above, in the case of not having undergone sex reassignment surgery, it is possible to achieve a trans pregnancy if hormonal treatments to stimulate ovarian reserve and facilitate trans pregnancy are withdrawn.
